Canvas Quizzes and Surveys allow you to create a series of questions for students to answer to assess their knowledge and skills or to provide you with feedback. As an instructor, you can create a quiz or survey directly in your Canvas course. Quizzes and Surveys can be graded or ungraded.

Step One: Create the Quiz or Survey

Step Two: Add the Quiz or Survey to a Module
Students can access published Quizzes, Assignments, and Discussions directly from the Canvas course menu, as well as through Modules.
If you create a quiz from the Quizzes page of your Canvas course, it is a best practice to always add the activity to a module. If you create a quiz directly from a module, that activity is automatically added to the module and accessible to your students when published.
To add a Quiz or Survey to a module:
- In the course navigation menu, click Modules.
- Click the Add Item (plus sign) to the right of a module's title to add a quiz or survey to it. The Add Item to Module window will appear.

Click the Add drop-down menu and select Quiz. A list of all of the Quizzes and Surveys in your course will be displayed.
- Select the quiz or survey that you would like to add to the module. (Please note that you can also create a new quiz or survey at this point by instead selecting [ New Quiz ] on this list.)
- Click Add Item. The quiz or survey you selected will now appear as the last item in the module.
